You need to measure the fabric overtop of the cushion as a flat piece. I would guess you would measure it out to be 18×18. You need two pieses, and give yourself a half an inch of wiggle room all the way around. If you do not have any trim on the edge, you can do it in once piece and not sew the end.
The piece must then be about 19" wide…
18+18+1 (the .5" on each end) + 6 (Flap) = 43" long
Sew the main pouch first (inside out) and continue down the flap to give it a finished edge all the way around.
Then the tricky part is to sew the flap but only to the closest piece of fabric and not all the way through both pieces.
Good luck

First you need know the width of fabric you plan to use.
Second does it have a pattern that will effect how it will look on the cushion..
Figuring on basic 45 inch width I would buy 1 and 1/4 yd fold end to end.open. the front and back of cushion. . Or if pattern design permits move over using only one 16 inch (1 inch seam allowance) double layer.
You will need 58-60 inch strip for the width around cushion. Possible enough from the part not used in the square cut for this.Try have seams on strip at sides so not so visable when complete.
It might help draw it out on paper first or take cushion with you to get better idea how design looks. I have even gone so far as make a paper pattern out of brown bag to be sure. Then lay and mark and cut fabric.
You will also need heavy duty thread or go over twice to make seams strong. First do basic long stitch, till get the fitting right to avoid ripping and redo. Then you can do normal stitch and double.
HAVE FUN.

have your foam 15X15 for a 14 inch cushion. this gives a nice snug fit
cut your top fabric 15X15 inches ( this gives you 1/2 an inch seam allowance)
For the back panel, cut one piece15Xat least 10 1/2 and cut the other at least 15X 11 1/2.
measure your cushion all the way around (or multiply 14 by 4) and add 1 inch to it for the 1/2 inch seam allowance for the sides (57 inches) – If the foam is 4 inches thick cut the fabric 4 inches wide for a nice snug fit. Unless you don’t want it to fit snug, then cut it 5 inches wide.
If the fabric is at least 30 inches wide from selvage to selvage, then you will need 25 inches–less than a yard
if the fabric is less than 30 ” wide from selvage to selvage then you will need 45 inches– less than 2 yards.–would be best to do the measurements yourself to double check mine.my math skills are out to lunch at the moment

1/2 yard.
